At Community Diagnostics, we take protecting your Personal Health Information (PHI) seriously! This means making sure your COVID-19 test results are securely transmitted via our PatientConnect portal. Below helps you understand the process and what to expect to access your results.
WHAT TO EXPECT
During collection, you are given the choice of receiving results via Text or Email or both. If you choose email, please note the email you provided so you know where to look for results.
Once available, you will receive a text or email (or both) from our automated system inviting you to login to our secure PatientConnect portal to retrieve your results while at the same time keeping your personal health information safe and protected. Simply follow prompts.

Notification via Email will appear in your inbox as below:

I received notification but cannot access my account.
Make sure the email address you provided at collection must be identical to the one you use to log into our Patient Portal and make sure you use the proper format to input your Date of Birth (DD/MM/YYYY).

It could be your sample is still in process but PLEASE CHECK Junk/Spam folder for email try search texts and email accounts for: firstname.lastname@example.org or PatientConnect before contacting customer support.
